The current Community-Based Service Delivery Model Guide for persons with intellectual disabilities is produced in the framework of the project funded by the European Union (EU) entitled “OASIS: Supporting the resettlement of persons with intellectual disabilities from the residential institution of Demir Kapija into community based supported living settings”.
The main objective of this integrated needs-based service delivery model Guide at community level is to contribute to the social inclusion of people with intellectual disabilities through the promotion of selfreliance and participation. The current guide aims at encompassing social and psychological, health, educational, employability promotion, and legal advisory services that will foresee the involvement and control of users in every step, including the development of personal resettlement plans.
The guide also includes an internal service quality monitoring & evaluation procedure for assuring and improving the quality of services and outcomes for people using the services of community based supported living settings.
